Raw psychological test data and assessment materials (e.g., test protocols, item-level scores, stimulus materials)
Quality of care information or records used solely for quality assurance
Records subject to legal privilege
Information whose disclosure could reasonably be expected to result in serious harm to your treatment, recovery, or to another person
If you are seeking raw test data for a specific purpose (e.g., re-assessment, legal proceedings), please contact the office directly to discuss your situation.

A fee of $30.00 covers the first 20 pages (includes retrieval, review up to 15 min, and response letter). Each additional page is $0.25.
You will receive a fee estimate before records are released.
Fees may be waived in exceptional circumstances.
Fee needs to be paid by e-transfer to payments@psychoedclinic.com
